In The Community
Arohanui aims, where possible, to enable patients to continue to live at home. Our team of specialist palliative care co-ordinators (PCC) monitor and support our community-based patients, maintaining close links with others involved, such as general practices and district nurses.
The role of the PCC is multi-disciplinary assessment of a patient and co-ordinating a patient’s ongoing palliative care needs in collaboration with other health care providers.
The family support team, consisting of social workers and a chaplain, offers personal and practical support for community-based patients, providing a wide range of assessment, brokerage, advocacy and counselling and pastoral support services.
